Decode Care is a nonprofit organization in the process of obtaining 501(c)(3) status, dedicated to developing and providing free health management technology for individuals with chronic conditions, seniors, and caregivers who face financial barriers to care. Founded in 2025, we believe that everyone deserves access to tools that can improve their health outcomes, regardless of their financial situation.
Decode Care is organized exclusively for charitable, educational, and scientific purposes. Our mission is to develop, maintain, and provide free access to health management technology for underserved populations, helping them better manage their health conditions and communicate more effectively with healthcare providers.

Our flagship initiative is the development of a mobile health management application designed to help users track medications, schedule appointments, and prepare for doctor visits with AI-generated health summaries. The application will be provided free of charge to individuals who need it most, with a focus on those managing chronic conditions, seniors navigating complex care requirements, and caregivers supporting loved ones.
We are also planning community support features and advanced AI insights to further improve health management capabilities for our users.
Decode Care was founded by Tiffanie Lyon, who currently serves as the organization's President. We are actively expanding our board of directors to include diverse perspectives and expertise, with the goal of building a team dedicated to our mission of improving health outcomes through technology.
